[plt-scheme] keywords aren't isomorfic to symbols?

I've added the missing documentation for scheme_intern_exact_keyword()
and scheme_intern_exact_char_keyword(). Meanwhile, the macros
SCHEME_KEYWORDP(), SCHEME_KEYWORD_VAL(), and SCHEME_KEYWORD_LEN() work
like the symbol counterparts.
