[plt-scheme] Errors in parser-tools documentation (299.100)

I've noticed two errors in the parser tools documentation
in 299.100:

1. return-without-pos is only defined in the lexer-src-pos
form, and not the lexer form as was the case in the 20x
implementations

2. The exception returned on lexer failure is not exn:read
but exn:fail:read.
