[plt-scheme] Web Server documentation reorganization

Hats off to you, Jay! It looks *great*.

A couple initial reactions:

- I think I preferred it when the front page table of contents included 
all the subsections; it results in less manual searching

- section 2.4 and I imagine 3.5 also probably don't need to be divided 
into multiple pages
