[plt-scheme] csv.plt - expand: unbound variable in module in: set-cdr!

Dear Lord Geoffrey,

What you are seeing in an incompatibility between csv.plt and v4.
Perhaps Neil can update csv.plt; I know he has received patches for
this update.
